  Client Profile

The Client Profile and Follow-up Notes is to track your progress.  You’ll note the space for an ID #.  Many fitness professionals assign their clients a number or simply use a membership number for reference.

Again, you’ll see the space for your information.  The sheet has spaces for specific body measurements every four (4) weeks.  Beneath that is a record for each weekly appointment.  We’ve provided space for crucial data.
